Existing TVUSD Students
Existing TVUSD Students need to download and return the following paperwork from the TVUSD web page. Click the appropriate link for TMS Returning Student to access forms.
- Student Data Sheet
- Student Health History
- Receipt of Annual Notification/Parent Signature Page
- Proof of Residency - Yearly requirement (Gas or Electric bill - The statement must be your primary residence address.)
- Proof of Tdap booster (7th through 12th Grade only)
Students new to TVUSD
New Students need to download and return the following paperwork from the TVUSD website. Click the appropriate link for TMS New Student to access the forms.
- New Student Information
- Student Data Sheet
- Student Health History
- Receipt of Annual Notification/Parent Signature Page
- Home Language Survey
- Release of School Records
- Residency Verification (Gas or Electric bill - The statement must be your primary residence address.)
In addition, New Students are required to provide the following:
- Birth Certificate
- Immunization Record, including Proof of Tdap booster (7th through 12th Grade only)
- Transcripts/Transfer or Withdrawal Grades
- Proof of Residency
- IEP/504 Plan (if applicable)
- GATE Placement (if applicable)
